peaqrx Model

One brand, two front doors. Her side leads with hormone therapy. Him side opens with sermorelin and NAD+ plus hair, with ED as an add-on, and moves to testosterone on Scale once the DEA position is clear. Costs are Karpa's: the monthly plan, a flat technology fee per completed consult, pharmacy product at catalog cost, pharmacy shipping per order, and Stripe at 2.9% + 30¢. Each patient is a basket: a lead product plus add-ons at the attach rates you set, with one consult and one shipment per order shared across the basket. You set the retail price. Patients from LLMA and the creator audience arrive at no cash cost; paid Meta patients carry the acquisition cost you enter.

Him add-ons and attach rate
Attach rate is the share of that side's patients who also take the add-on. Click a ledger row to make it the lead.
Add-ons ship
Three pharmacies fill the catalog. Items from the same pharmacy ship together; a second pharmacy means a second shipping charge.
Refill consult, non-controlled products
A consult at signup, then every 3 to 6 months depending on the product. Testosterone is every 90 days regardless; the model fixes it there. Karpa bills $30 async or $50 sync per consult, plus $10 when the visit covers more than one service, added here at the probability a patient has an add-on. One consult covers the whole basket.

Prepay discount%
Prepaid matches the 90-day pharmacy order, cuts Stripe to one charge, and collects three months of cash before the first fill ships.
Spreads intake, labs and acquisition cost across the patient's lifetime
Default $0: Karpa's $30 to $50 consult is absorbed as the acquisition cost for owned-audience patients. If charged, label it an intake or program fee at checkout, not a doctor's fee. Hims and Hers bundle it; Evernow charges $150 per visit outside membership.
Karpa (Darick, 2026-10-29): shipping is billed to the patient through the Karpa system at the pharmacy's cost, $20 for tablets and capsules, $30 for cold chain, overnight only for short shelf-life items. Karpa recommends baking it into the treatment price and marketing it as included, which is what 0 here means: the shipping column still comes out of your retail. Enter the tier rate to charge it as a separate line instead.
Charged once on TRT and HRT. The TRT panel costs $121.75, the HRT panel $128.25.

Retail / mo figures are peaqrx working prices, not Karpa's. ED, hair and skin lines were set on 2026-10-30 to the median of four Karpa client sites selling the identical SKU (sildenafil $107, finasteride $81, tretinoin $116, GHK-Cu $179 and so on); peptides already matched that cohort; GLP-1 lines are above it and unchanged. Karpa's sheet leaves the retail column to the partner. Net / mo is retail (after any coupon and prepay discount) plus the consult fee you charge, minus drug, shipping, Karpa's consult fee, Stripe, the spread lab cost net of the lab fee, and the spread paid acquisition cost. With 3-month billing, retail is collected a quarter at a time; the ledger still shows it per month. Break-even alone is the patient count that covers the plan fee this product requires plus other fixed cost, if the whole base were on this one product.

Clean lane is commercial generics plus compounded items with no regulatory flag: what peaqrx launches with. Compounded testosterone, enclomiphene and the peptides marked pending are on Karpa's sheet but stay off the peaqrx menu.

Testosterone needs the Scale plan and a separate controlled-substance addendum. The DEA telemedicine flexibility that allows it without an in-person visit expires December 31, 2026 unless extended. Karpa's position (2026-10-29): no change expected before the deadline, TRT expected to remain available with possible added steps such as an in-person evaluation or extra registration, handled by the Elite Care providers; no commitment on whether new TRT intakes continue if the flexibility lapses. HRT needs Growth and has no such limit.

Consults follow the product: a signup consult, then every 90 days for testosterone and every 3 or 6 months for everything else, one per basket. Not modeled: refunds on declined intakes, chargebacks, the Launch plan's three-program limit, state exclusions, and Karpa's $1,997 start-up fee (self-built site, per Darick 2026-11-03; $2,997 if Karpa builds it) plus the $3,000 LegitScript package.
